Getting Started

Please read before submitting your order:

After you submit your order you will receive via the e-mail provided your new patient paperwork and lab sheet for your blood tests.  You will also receive detailed instructions about the lab locations available and how to find them.  We use both LabCorp and Quest Diagnostics.

Your completed labs are usually returned to us within 4-5 business days.  Once received by us, one of the nurses will contact you to schedule an appointment for you to see the doctor.  Please allow 1 full business day for processing and paperwork.  Your initial consult fee is $650 and includes your labs, your consultation with the doctor, your physical exam and diagnosis.  There is no co-pay or insurance due during your visit.  Pricing for monthly plans can be found on the PRICING PAGE of this site.

	I understand that Doctors Age Management reserves the right to treat and diagnose me and I am applying for treatment.  I understand that Doctors Age Management may choose not to treat me due to underlying medical conditions or non compliance to the treatment protocol.  By submitting my payment I understand that this action does not guarantee me treatment and I understand by submitting this information I must be diagnosed with a condition requiring treatment in order to be treated.  If a condition does “not” exist or warrant treatment I understand I will not be treated. In the event Doctors Age Management chooses not to treat you, your initial fee “will be refunded” within 3 business days, less a $50 administrative and doctors review surcharge.
